Notice to Parents and Guardians:

This form grants permission to the organization to capture, publish, and transmit media content (photographs, live streams, video footage, and audio) of your player for team administration, promotional, scouting, and digital media purposes.

3. Terms of Authorization & Media Grant

By signing below, I hereby grant the Missoula Mud Dawgs Baseball Organization, its coaches, tournament organizers, affiliated leagues, media staff, and authorized representatives full permission to record, photograph, broadcast, and publish the likeness, voice, name, and athletic performance of the minor player named above.

• Scope of Use: Media captured may be used for official team websites, social media platforms (e.g., Instagram, Facebook, YouTube), live-streaming services (e.g., GameChanger, AthletesGoLive), tournament highlight reels, college recruiting profiles, print marketing, and local news press releases.

• Ownership & Compensation: I acknowledge that all photographs, digital images, raw video, and audio recordings remain the sole property of the organization. I waive any right to financial compensation, royalties, or inspection/approval of final published media products.

• Safety & Privacy: The organization agrees not to publish personal sensitive information (such as home addresses, phone numbers, or academic records) alongside media content without explicit additional written consent.

4. Consent Selection (Check One)

  I DO NOT authorize the use of my child's photo or media. (Note: Players may still appear inadvertently as background subjects in general game footage or opponent broadcasts).
